There are a couple of pictures posted on this forum that you can print out and make one, other than that someone here might be able to set you up.
I have used a motion pro wheel many times, and it is available at your local shop. You just have to use it on the other side of the crank. I picked up a kent moore/cannondale wheel off of ebay a couple months back too, so keep your eye out there if you are set on getting the right one.
SPX is the parent company for kent moore, OTC, and many other speciality tool companies. You may be able to check with a tool truck or SPX directly. I asked my tool salesman and he was going to check into getting the original wheel from Kent Moore, but I never heard back. I have called SPX directly on a specialty wrench for the 5th gear nut on a NV4500 transmission and was able to get it through them since there wasn't a dealer locally.
