Helicoil broken

I pulled the motor this weekend for a couple of reasons. The main being that I wanted to inspect all of the bolts that have been famous for coming loose, essentially everything that pertains to the stage 8 update.
Well, while taking the first bolt out, the helicoil came with it. This can not be right. Has anybody else had this happen to them? I stopped right there and did not do anything else.
Can those helicoils be replaced? If so, does anybody know the size? Why did the company use helicoils as opposed to tapping the case and just drilling into the case?
Basically I am at a total stand still here, and don't want to do anything else before I find a solution to this problem.
